I see in the ERD you have MailingMembers linked to Appointments but I don't see any interface layouts to link the two together in the UI so that the merge tokens you've set up will grab data from the Appointment itself. Is there a mini-window (or something) somewhere that I've missed that links these Appointments to Mailings in the UI?

That link is made when you create a mailing from the context of a found set of appointments: from the appointment list click the "letter" button and your found set will be sent to the mailing engine using the appointment's contact as the recipient and then any merge fields from the appointment will be active.
